La teriflunomida es un tratamiento modificador de la enfermedad oral aprobado para el tratamiento de las formas recurrentes de MS. En el modelo preclínico del virus de la encefalitis murina de Theiler para la MS, el fármaco demostró una mayor tasa de eliminación viral frente al vehículo placebo. Además, la teriflunomida inhibe la infección lítica por EBV in vitro. Objetivo: 1. Evaluar la respuesta humoral frente a EBV y HHV-6 antes del tratamiento con teriflunomida y 6 meses después. 2. Correlacionar la variación de la respuesta humoral frente a EBV y HHV-6 con la respuesta clínica y radiológica tras 24 meses de tratamiento con teriflunomida. 3. Analizar la utilidad de diferentes datos demográficos, clínicos, radiológicos y ambientales para identificar biomarcadores tempranos de respuesta a la teriflunomida. Métodos: Se reclutó a un total de 101 pacientes con MS (62 mujeres; edad media: 43,4 años) con un suero previo al inicio de la teriflunomida y otra muestra de suero 6 meses después. Un total de 80 habían sido tratados durante al menos 24 meses, 13 habían interrumpido la teriflunomida antes de los 24 meses, y 8 se encontraban actualmente en tratamiento con teriflunomida pero con menos de 24 meses de seguimiento. Analizamos los niveles de los títulos de anticuerpos virales mencionados anteriormente en muestras de suero mediante kits comerciales de ELISA, y los niveles séricos de la cadena ligera de neurofilamentos (Nf-L). Resultados: Los títulos de anticuerpos antivirales disminuyeron para EBNA-1 IgG (74,3%), VCA IgG (69%), HHV-6 IgG (60,4%) y HHV-6 IgM (73,3%) tras 6 meses de teriflunomida. In the preclinical Theiler’s murine encephalitis virus model of MS, the drug demonstrated an increased rate of viral clearance versus the vehicle placebo.
