    Injury Incidence Increases after COVID-19 Infection: A Case Study with a Male Professional Football Team.

    Background: The SARS-CoV-2 virus disease has caused numerous changes in sports routines in the last two years, showing the influence on an increase in sports injuries. The aim of this study was to prospectively analyze the incidence and characteristics of injuries in male professional football players diagnosed with COVID-19 when they return to play after recovering from this illness. Methods: Injury characteristics of professional male football players were recorded for the 2020–2021 season following the international consensus statement from the International Olympic Committee (IOC). SARS-CoV-2 infection in the football players was certified by PCR analysis. Injury epidemiology was compared in players infected by the SARS-CoV-2 virus before and after being diagnosed with COVID-19. Results: 14 players (53.8%) were diagnosed with COVID-19 during 2020–2021 season and 12 (46.2%) were not infected (controls). Only three (21.4%) had suffered an injury before being diagnosed with COVID-19. Eleven players (78.6%) had injuries after being diagnosed with COVID-19 (p < 0.001). Among the players diagnosed with COVID-19, injury incidence increased on their return to play after the infection (3.8 to 12.4 injuries/1000 h of exposure, p < 0.001). Additionally, injury incidence during training (10.6 vs. 5.1 injuries/1000 h of exposure, p < 0.001) and matches (56.3 vs. 17.6 injuries/1000 h of exposure, p < 0.001) was ~two-fold higher on return to play after COVID-19 compared to controls (33.4 vs. 17.6 injuries/1000 h of exposure, respectively, p < 0.001). Conclusions: Injury incidence in professional football players who had been infected by the SARS-CoV-2 virus significantly increased compared to the injury rates that these same players had prior to the illness. Additionally, the injury incidence was higher when compared to players who were not infected by the SARS-CoV-2 virus during the season, especially during matches

    Effect of p-Synephrine on Fat Oxidation Rate during Exercise of Increasing Intensity in Healthy Active Women.

    p-Synephrine is the principal alkaloid of bitter orange (Citrus aurantium). Several recent investigations have found that the intake of 2–3 mg/kg of p-synephrine raises fat oxidation rate during exercise of low-to-moderate intensity. However, these investigations have been carried out only with samples of male participants or mixed men/women samples. Therefore, the aim of this investigation was to study the effect of p-synephrine intake on fat oxidation during exercise of increasing intensity in healthy women. Using a double-blind, randomized experiment, 18 healthy recreationally active women performed two identical exercise trials after the ingestion of (a) 3 mg/kg of psynephrine and (b) 3 mg/kg of a placebo (cellulose). The exercise trials consisted of a ramp test (from 30 to 80% of maximal oxygen uptake; VO2max) on a cycle ergometer while substrate oxidation rates were measured at each workload by indirect calorimetry. In comparison to the placebo, the intake of p-synephrine increased resting tympanic temperature (36.1 ± 0.5 vs. 36.4 ± 0.4 °C p = 0.033, d = 0.87) with no effect on resting heart rate (p = 0.111) and systolic (p = 0.994) and diastolic blood pressure (p = 0.751). During exercise, there was no significant effect of p-synephrine on fat oxidation rate (F = 0.517; p = 0.484), carbohydrate oxidation rate (F = 0.730; p = 0.795), energy expenditure rate (F = 0.480; p = 0.833), heart rate (F = 4.269; p = 0.068) and participant’s perceived exertion (F = 0.337; p = 0.580). The maximal rate of fat oxidation with placebo was 0.26 ± 0.10 g/min and it was similar with p-synephrine (0.28 ± 0.08 g/min, p = 0.449, d = 0.21). An acute intake of 3 mg/kg of p-synephrine before exercise did not modify energy expenditure and substrate oxidation during submaximal aerobic exercise in healthy active women. It is likely that the increase in resting tympanic temperature induced by p-synephrine hindered the effect of this substance on fat utilization during exercise in healthy active women.

    Study of frequency and type of adverse analytical findings in the different disciplines of aquatics.

    We aimed to analyze the number and distribution of doping control tests in which a banned substance was reported (i.e., adverse analytical finding) in aquatics. The analysis was performed by using the data provided by the WADA Testing Figure Reports from 2015 to 2019. A total of 79,956 doping control tests were analyzed. Sprint swimming, middle-distance swimming and water polo were the disciplines with the highest number of doping control tests. However, there were no differences in the frequency of adverse findings among disciplines (overall, ∼0.56%, from 0.13 in artistic swimming to 0.76% in sprint swimming). Sprinters and long-distance swimmers presented a higher frequency of beta-2-agonists than the remaining aquatic disciplines (p < 0.05). These results indicate that the type of prohibited substances employed is strongly influenced by the intrinsic characteristics of each aquatic discipline.

    Outcomes of adverse analytical findings in individual and team sports.

    The aim of this investigation was to describe the outcomes of the adverse analytical findings in different Olympic sports. The data included were gathered from the World Anti-Doping Agency Anti-doping Rule Violations Reports (2013–2017). Weightlifting (78.1 ± 9.4%) wrestling (73.2 ± 18.5%) and volleyball (68.3 ± 18.7%) were the sports with the highest proportion of cases that ended in an antidoping sanction. Gymnastics (45.1 ± 10.1%), triathlon (32.6 ± 11.9%) and shooting (29.9 ± 14.1%) were the sports with a higher frequency of cases that were not sanctioned due to medical reasons. Gymnastics (22.4 ± 18.4%), boxing (23.2 ± 16.0%) and taekwondo (17.3.1 ± 16.4%) presented the highest proportion of cases that are still pending resolution. The proportion of cases that ended in no sanction was higher in fencing (26.2 ± 22.7%), skating (23.6 ± 35.1%) and tennis (18.6 ± 26.5%). These results indicate that the sanctions derived from antidoping rule violations were not uniform in all sports disciplines.

    No diurnal variation is present in maximal fat oxidation during exercise in young healthy women.

    Maximal fat oxidation during exercise (MFO) and the intensity that elicits MFO (Fatmax) seems to show a diurnal variation in men, which favours an increased performance in the afternoon than the morning. At present, it remains unknown whether the observed MFO and Fatmax diurnal variation in men is also present in women. Therefore, the current study examined the diurnal variations of MFO and Fatmax in women. Nineteen healthy women (age: 26.9 ± 8.7 years, maximum oxygen uptake: 39.8 ± 6.5 ml/kg/min) participated in the study. MFO and Fatmax were determined by a graded exercise test in cycloergometer using a cross-over design performed on two separate daytime schedules, one conducted in the morning (8am–11am) and one in the afternoon (5pm–8pm). Stoichiometric equations were used to calculate fat oxidation rates. There were no significant differences between MFO-morning and MFO-afternoon (0.24 ± 0.10 vs. 0.23 ± 0.07 g/min, respectively; P = 0.681). Similarly, there was no significant differences between Fatmax-morning and Fatmax-afternoon (41.1 ± 4.7 vs. 42.6 ± 5.5% of maximal oxygen uptake, respectively; P = 0.305). These results persisted after controlling for fat mass percentage (all P > 0.5). In summary, the main finding of the present study was that MFO and Fatmax were similar independent of the time-of-day when the exercise test is performed in healthy women. These results have important clinical implications since they suggest that, in contrast to what was found in men, MFO and Fatmax show similar rates during the course of the day in women.

    Sport-Specific Use of Doping Substances: Analysis of World Anti-Doping Agency Doping Control Tests between 2014 and 2017.

    Background: In recent years, there has been a solid effort across all sports organizations to reduce the prevalence and incidence of doping in sport. However, the efficacy of current strategies to fight against doping might be improved by using anti-doping polices tailored to the features of doping in each sport. Objectives: The aim of this investigation was to analyze the substances more commonly found in doping control tests in individual and team sports. Material and Methods: The publicly accessible Testing Figures Reports made available by the World Anti-Doping Agency, were analyzed from 2014 to 2017. Results: The most commonly detected groups of banned substances were anabolic agents and stimulants but the distribution of adverse findings per drug class was very different depending on the sports discipline. Weightlifting, athletics, rugby, hockey and volleyball presented abnormally high proportions of anabolic agents (p = 2.8 × 10−11). Cycling, athletics and rugby presented atypically elevated proportions of peptide hormones and growth factors (p = 1.4 × 10−1). Diuretics and masking agents were more commonly found in boxing, wrestling, taekwondo, judo, shooting, and gymnastics than in other sports (p = 4.0 × 10−68). Cycling, rowing, aquatics, tennis, gymnastics and ice hockey presented abnormally high proportions of stimulants (p = 1.8 × 10−5). Conclusions: These results indicate that the groups of banned substances more commonly detected in anti-doping control tests were different depending on the sports discipline. These data suggest the prohibited substances used as doping agents might be substantially different depending on the type of sport and thus, sports-specific anti-doping policies should be implemented to enhance the efficacy of anti-doping testing.

    Effects of CYP1A2 and ADORA2A Genotypes on the Ergogenic Response to Caffeine in Professional Handball Players.

    Previous investigations have found that several genes may be associated with the interindividual variability to the ergogenic response to caffeine. The aim of this study is to analyze the influence of the genetic variations in CYP1A2 (−163C  > A, rs762551; characterized such as “fast” (AA genotype) and “slow” caffeine metabolizers (C-carriers)) and ADORA2A (1976T  > C; rs5751876; characterized by “high” (TT genotype) or “low” sensitivity to caffeine (C-carriers)) on the ergogenic response to acute caffeine intake in professional handball players. Thirty-one professional handball players (sixteen men and fifteen women; daily caffeine intake = 60 ± 25 mg·d−1) ingested 3 mg·kg−1·body mass (bm) of caffeine or placebo 60 min before undergoing a battery of performance tests consisting of a countermovement jump (CMJ), a sprint test, an agility test, an isometric handgrip test, and several ball throws. Afterwards, the handball players performed a simulated handball match (2 × 20 min) while movements were recorded using inertial units. Saliva samples were analyzed to determine the genotype of each player for the −163C  > A polymorphism in the CYP1A2 gene (rs762551) and for the 1976T  > C polymorphism in the ADORA2A gene (rs5751876). In the CYP1A2, C-allele carriers (54.8%) were compared to AA homozygotes (45.2%). In the ADORA2A, C-allele carriers (80.6%) were compared to TT homozygotes (19.4%). There was only a genotype x treatment interaction for the ball throwing from 7 m (p = 0.037) indicating that the ergogenic effect of caffeine on this test was higher in CYP1A2 AA homozygotes than in C-allele carriers. In the remaining variables, there were no genotype x treatment interactions for CYP1A2 or for ADORA2A. As a whole group, caffeine increased CMJ height, performance in the sprint velocity test, and ball throwing velocity from 9 m (2.8–4.3%, p = 0.001–0.022, effect size = 0.17–0.31). Thus, pre-exercise caffeine supplementation at a dose of 3 mg·kg−1·bm can be considered as an ergogenic strategy to enhance some neuromuscular aspects of handball performance in professional handball players with low daily caffeine consumption. However, the ergogenic response to acute caffeine intake was not modulated by CYP1A2 or ADORA2A genotypes.

    Acute caffeine supplementation enhances several aspects of shot put performance in trained athletes.

    The aim of this investigation was to determine the effect of a moderate dose of caffeine (3 mg/kg/b.m.) on muscular power and strength and shot put performance in trained athletes. Methods. Thirteen shot putters (eight men and five women) participated in a double-blind, placebo-controlled, randomized experiment. In two different trials, participants ingested either 3 mg/kg/b.m. of caffeine or a placebo. Forty-five min after substance ingestion, athletes performed a handgrip dynamometry test, a countermovement jump (CMJ), a squat jump (SJ), and a maximum-velocity push-up. The athletes also performed three types of throws: a backwards throw, a standing shot put and a complete shot put. Results. In comparison with the placebo, caffeine ingestion increased CMJ height (32.25 ± 7.26 vs. 33.83 ± 7.72 cm, respectively; effect size (ES) = 0.82, p = 0.012; +5.0%;) and SJ height (29.93 ± 7.88 vs. 31.40 ± 7.16 cm; ES = 0.63, p = 0.042; +6.4%) and distance in the standing shot put (10.27 ± 1.77 m vs. 10.55 ± 1.94 m; ES = 0.87, p = 0.009; +2.6%). However, caffeine ingestion did not increase strength in the handgrip test, power in the ballistic push-up, or distance in the backwards throw (all p > 0.05). Shot put performance changed from 11.24 ± 2.54 to 11.35 ± . 2.57 m (ES = 0.33, p = 0.26; +1.0%), although the difference did not reach statistically significant differences. Caffeine ingestion did not increase the prevalence of side effects (nervousness, gastrointestinal problems, activeness, irritability, muscular pain, headache, and diuresis) in comparison with the placebo (p > 0.05). Conclusion. In summary, caffeine ingestion with a dose equivalent to 3 mg/kg/b.m. elicited moderate improvements in several aspects of physical performance in trained shot putters but with a small effect on distance in a complete shot put.

    Validación de una encuesta para determinar la prevalencia en el uso de suplementos en deportistas de élite españoles.

    Introducción: el uso de suplementos deportivos es una estrategia generalizada entre los deportistas de alto rendimiento. Sin embargo, no existe ningún cuestionario validado en castellano para determinar la prevalencia en el uso de suplementos deportivos en deportistas. Objetivo: el objetivo de esta investigación fue validar un cuestionario en castellano para estudiar la prevalencia del uso de suplementos deportivos. Material y métodos: en una primera fase, el cuestionario fue diseñado para recabar información sobre la frecuencia en el uso de cinco categorías de suplementos (estimulantes, potenciadores del rendimiento, suplementos para control de peso, “recuperadores” y medicamentos). Posteriormente, el contenido del cuestionario fue validado por un grupo de seis expertos que valoraron la pertinencia y claridad de cada pregunta del cuestionario en escalas de 1-10 puntos. La versión final del cuestionario fue completada en dos ocasiones separadas por un mes (test-retest) por un grupo de 39 atletas de élite, con el objetivo de verificar la reproducibilidad en las respuestas a este cuestionario. Resultados: la versión final del cuestionario, obtenida tras la validación de contenido, tiene 81 preguntas que recaban información sociodemográfica y sobre motivos, frecuencia y condiciones de compra de suplementos deportivos. En el test-retest se comprobó una alta congruencia en todas las preguntas del cuestionario, sin diferencia estadística en ninguna de las respuestas en el test-retest. Conclusión: el cuestionario derivado de este estudio cumple con los parámetros de validez y fiabilidad necesarios para la obtención de información vinculada al tipo y frecuencia en el uso de suplementos deportivos.

    Frequency and type of adverse analytical findings in athletics: Differences among disciplines.

    Athletics is a highly diverse sport that contains a set of disciplines grouped into jumps, throws, races of varying distances, and combined events. From a physiological standpoint, the physical capabilities linked to success are quite different among disciplines, with varying involvements of muscle strength, muscle power, and endurance. Thus, the use of banned substances in athletics might be dictated by physical dimensions of each discipline. Thus, the aim of this investigation was to analyse the number and distribution of adverse analytical findings per drug class in athletic disciplines. The data included in this investigation were gathered from the Anti-Doping Testing Figure Report made available by the World Anti-Doping Agency (from 2016 to 2018). Interestingly, there were no differences in the frequency of adverse findings (overall, 0.95%, range from 0.77 to 1.70%) among disciplines despite long distance runners having the highest number of samples analysed per year ( 9812 samples/year). Sprinters and throwers presented abnormally high proportions of adverse analytical findings within the group of anabolic agents (p < 0.01); middle- and long-distance runners presented atypically high proportions of findings related to peptide hormones and growth factors (p < 0.01); racewalkers presented atypically high proportions of banned diuretics and masking agents (p = 0.05). These results suggest that the proportion of athletes that are using banned substances is similar among the different disciplines of athletics. However, there are substantial differences in the class of drugs more commonly used in each discipline. This information can be used to effectively enhance anti-doping testing protocols in athletics.
